Apparently this is not a piston exiting a bike, but it's a nitrous backfire. It blew the intake out.

If you think about it, there is no way a piston could exit the motor. It would have to make it (to hit his hand) past the head, valvetrain, cams, valve cover, fuel tank or the chassis. Neither of which is very likely. Even to get a con rod up that far would be an amazing feat, and that's all that'd be left from the journy is a chunk of cast iron (or alum/ti for racing).

The bike part that flew out came from under the tank on the other side of the bike. The tank got blown up a few inches and hit him in the ribs. That's why he immediately grabbed his chest with his left hand before deciding to jump for it.
