You'd think that because Underdog was being examined by doctors, they would've asked him to undress, and then... hello. Could THIS SWORD be the thing that's causing his pain? It's really a no-brainer here.
There is one major flaw in this episode. Riff Raff's first attempt to steal the sword collection caused the train wreck and Underdog's pain. The sword's were recovered and placed on display in the San Francisco museum where Riff Raff successfully stole the collection. Before they were put on display at the SF Museum, they should have noticed one was missing.

When I was 6 or 7 years old, I knew that it was utterly stupid that underdog, who is bulletproof, explosion proof, etc.... was caused such pain by a stupid sword down his uniform. I think I quit watching it after this.
