this is 100% the fault of the service guy that didnt realise the liquid withdrawl valve was not working, if it was me i would have used my boom truck to pick that thing up and put it in the middle of a field if i couldnt get it stopped, if that ever happens to you, sue your pump hose with the valve open to let pressure out while you attah the hose, then close it.
lol tyty was right is saw the shit right after it happened and the dam blue icecream machine dint fuckin budge an ambulance was torn in half but the mr.freeze had only scratches....wtf
Incredible how this tragedy unfolded from a small act into a catastrophe. It just goes to show that with all those people around and some with specialist training in saving lives that common sense doesn't always prevail and that there was plenty of time to save those that were killed...if only someone had of realised sooner to order an evacuation.
Very tragic, on any gas leak you evacuate people right away, you don't act like it's no big deal that your in standing around in a major amount of propane... They should have yelled at them from a distance and got out of there and set up command at a safe distance.
This is so sad...thank you CSB for posting these videos so us student engineers can learn from the mistakes of others and remember them so that they are never repeated.
i work for a propane company and all of our techs are gold certified as should every other propane tech people dont realize how dangerous it can be. Every precaution should be taken
i have filled this tank when i worked for ferrell gas. first of all the tank should have been ten feet away. there are alot of faulty tanks out there that need to be scrapped instead of just painted!!!
FM Lewis is very pro-training and progressive when it comes to issues related to making the fire service better. I see major departments every day on the news with bunker coats open when dealing with fuel spills. We, as firefighters, need only to look to ourselves. We handle so many, "BS" type calls we become complacent. I just wonder how many who posted comments actually live what they preach 24/7, or just when it is convenient to become the champion of safety.
The "training" that needed to take place was with the maintenance (vendors) that were servicing the tank. With proper training of the person causing the incident, everything else could have been avoided.
First, the lead service technician should not have left the scene. Second, proper training of the apprentice tech should have happened before allowing him onsite.
A complete evacuation of the building and surrounding area should have taken place the moment the gas was released.
The CBS suggested that the state should mandate more than 4 hours of training.
As an officer of a Very BIG fire department (845 members),I have to agree with the CBS to the needed training.
A fire marshal should get this training mandated for his fire departments and see that it's done. As a good training officer, you should show them the door and make sure they walk through it!
As a training officer for a small VFD I can attest to the difficulties to getting members trained. Like it was said in the "Matrix" "I can only show you the door, it is up to you to walk through it." You can schedule training, but it is up to the people to come to it. As far as training goes, it's not his fault. It is the fault of the individual FF and the scene commander for letting the FFs endanger themselves due to lack of training.
off topic, did this town originate from a Belgian colony? (Gent is a Belgian city) I know New York used to be New Amsterdam until it went British and renamed after York etc. but didn't know Belgians had colonies in the new world in the colonial era.
PS I don't understand 1 thing though, the people and employees in the store. If I smell gas I leave the building, no one needs to ask me to leave, common sense is telling me to go.
Good video though. Condolences to the family members left behind.
dumb!!

Beyond anything else, I cannot understand why the employees in the store didn't leave/evacuate.

Does anyone know the severity to which the convenience store personnel were injured?
dunderdolt 2 years ago
I was in that gas station less than an hour before this happened...
thats a scary thought.
And the only thing in the store that survived was an ice cream cooler.
so if you know somethings about to explode, jump in an ice cream cooler haha.
sorry thats really no laughing matter...
and plus you could feel that from more that 25 miles away, imagine the people who lived close!

I have massive amounts of flammable gas escaping, forming an explosive mix with the air. Gee mister, whatever should I do?
This is so unbelievably strange. Who would lock themselves in a building that is clearly filled with a flammable gas?
How come these people did not run to a save distance in total panic?

In West Virginia it is not the State Fire Marshal that mandates and/or handles training - it is the West Virginia State Fire Commission.

Sterling Lewis Dropped the ball for Vol. fire dept. training.
for an ex. Vol. Fire Chief, he should have had more training and checks and balances for Vol. firefighters to make sure that training is up to par!
