You can get a Gentoo boot cd of the web, once it's installed your fan problems should be a thing of the past, had the exact same problem last week and it fixed it.
It's the cheap fan controllers and fans they are putting in alot of newer components. I just suffered a fan loss on my nvidia card and it did the same exact thing, so I ordered a replacement fanless heatsink.
Sounds like it's overheating and the fan speed is jumping around because of the skittish controller settings.
My ThinkPad from 2003 isn't doing that(yet), but my Dell from 1998 did until I cleaned out the heatsink... just last week my neighbour's P4m Toshiba from 2004 was doing that so I cleaned it out and now it's silent again.
Either pay Apple $200 to pick the gunk out of the puny heatsink, or do it yourself.
Do you have any kind of plan that allows you go get free maintenance? I don't have a local apple store, so I don't know if they will take a look at your stuff for free... I agree that it sounds like dirty fans.. Maybe you can have them cleaned or replaced. After you get it fixed you should probably invest in a usb powered cooler. It never hurts.
My macbook pro was doing the same thing within a year of purchasing it. I took it into apple and the first thing the guy tried was removing the battery and cleaning the heat sinks which are a common area where dust gathers and reduces the effectiveness of the heat sinks. I took it home and it still made the overclocked fan sounds so I took it back in and the guy ordered two brand new fans which came in a couple days later and they were installed that same day. I haven't had a problem since.

fans are extremely cheap if you need to replace it. I wouldnt suggest taking it to an apple store. they will charge you an outrageous amount of money for something so miniscule. IF you need to replace the fan i would expect to pay no more than 30-40$. I would hate to see someone pay as the previous poster suggested pay a couple hundred dollars. it is totally unnessecary. its extremely simple and easy to fix.
it obvious what is going on, your fan needs cleaned out. YOU may not be doing anything to it physically, but with it powered on, it is overheating is now unstable. After a couple of years, laptop fans accumulate a lot of dust. This dust can clog up your heatsink and heating vent, thus heat can't escape. Its actually an extremely easy fix. If you do this or have someone do it for you, after it is cleaned make sure to have new thermal grease applied to your processor before reassembling it.
I agree, recent SMC, but i doubt thats going to be a perm fix. You're going to have to take it to Apple store and see what they say. it'll probably be a few hundred dollars to fix those fans, but it's time for an upgrade!! If you can't afford either an upgrade of the fix from apple right now, I'd get one of those external USB laptop cool pads to help if that issue is causing overheating...
mines 3 yrs old its done this b4 but not that loud i can never really hear the fan it sounds like it could be ur cd drive to but its between fan and cd drive
ok do this, instead of buying a new macbook for 899-1299/get a new battery for 130; get a new apple care for 60 bucks and take it to the apple store to get it fixed. They say any fan problems....they can help calm it down.
Sorry to hear that, you should try turning off your macbook and take the battery out. Then put it back in and start it up again. For some reason it does fix a lot of problems on the Macbook.
if i were you i would try a clean install of snow leopard and if that didnt fix the problem i would have to say there is something wrong on the inside. my friends fan use to do that all the time and apple fixed it for him.
you probley shouldnt have it flat down like lean it in a slope so the fan doesnt probley do that umm put a book in the back or somthing and see if the fan goes different .. ( hope it helps )

OK heres the easiest and best fix that there is:
DON'T GET A MACBOOK (try some other brand that doesn't charge $4000000 for a repair simply because their systems are unique and/or are hard to fix on your own)
DRBUK says his was doing something like that when he installed snow leopard, maybe its pushing the hardware some where on the system to far and its allowing it to do that.
