He's using a TV flyback, oscilating at 15,000 hz to 20,000 hz - the current is in range from 4 - 8 ma, and most likely 45-50 KV.
When electricity oscilates so quickly it produces magnetic fields which propage the current around the conductor vs within the conducter.
This is not rocket science, any - one who hobbies in electronics knows this. I have a video of me, doing very similer.

He claims he wasn't hurt the first time he was electrocuted. That, obviously, is open to speculation, but assuming the documentary isn't a complete sham, the man's heart is in perfect condition despite all the electrical abuse he's taken which proves it's not just pain resistance.

if you pay attention, he's using a plastic crate to isolate himself from ground. no doubt that hurts, but there's not enough current to kill him.

I'm no expert when it comes to electricity but it's obvious that there's enough electricity flowing through him to light that florescent light. Also, he'd be experiencing muscle contractions and paralysis, none of which he seems to be experiencing.
Also, if you watch the entire documentary you'll see that upon hearing this argument from the physicists he sticks his hands into a 240 volt power breaker without the milk crate. He also performs at the end of the show without the milk crate.
