I may be wrong, but I think it actually is from the Sailor Moon show. It's not the music box theme, but it may be the background music that plays when this villain is onscreen at some time. Or possibly it's from one of the live action Sailor Moon musicals, which are separate from of the original anime. I am almost positive, however, that I have seen this midi on Sailor Moon websites before, so I'm thinking your first lead was right. I'm a SM fan, so I'll see if I can find out for you.

I think I saw that site forever ago. I think it's originally called "Tonatiuh". At least, I've found this same song in midi form with that title.
