Reminds me of Eye of the Beholder from The Twilight Zone. I heard about this ad from a TV Tropes section about advertising that also qualifies as nightmare fuel, so congratulations on being terrifying enough to be mentioned there.
I think its strangeness and darkness makes this awesome....though it doesn't even matter what it's advertising, it's like a cool, creative short film all on its own.
I saw this in the theatre before the previews started. Can't remember what movie anymore (something from 2007, obviously). I shat a brick. Seriously, not cool. I think it fails as a commercial (or campaign promo, whatever), because you don't want to horrify your consumers, do you? On the other hand, good job writing something...memorable. It is cool that the writer of the spot uploaded it...it's fun to hear the behind-the-scenes details. Love to see the actual shooting script.

@superkeijikun This wasn't really a car commercial (obviously), it was more a commercial for a website that promoted a car (the Scion xB). Car commercials are a dime a dozen. We were trying to do something interesting and different to get the public's attention and engage them a bit more than just another car commercial. Scion used to take a lot of risks like this with their ads when I was a creative at their ad agency. They still do interesting stuff, but not risky like this campaign was.
Here's a hint for making car commercials: Instead of showing people getting square heads at a deranged hospital, why not show people with square heads driving the car you're advertising?

Ha. Guess we did our job (I came up with the idea and wrote the script, but it was the director, actors and other people that made it come to life that made it freaky as it is). I think I had nightmares from the place we shot it. We were in the basement of an old hospital in Romania. There were long hallways with huge rusty pipes running along the ceiling. The place was filthy. Yet there were actually some people living in little rooms down there! Reality is creepier than fiction.
Goddam this commercial! I saw this like, 2 years ago, and was so freaked out, and had no idea what it was advertising or anything! Actually, looking at it now it's kind of funny. Mainly because the two guys actually don't mind it, but the normal guy is the freaked one.

Directed by James Rouse, who is without a doubt the most talented director I've worked with. An incredibly hard worker who took my script and made it way better, which is the sign of a great director.
I saw this, I thought it was an anti-drug PSA, never in a million years would I have thought this was related to cars.
