The apple symbolizes the forbiddin fruit in the garden of Adam and Eve's. It being in the palm of a hand symbolizes the forbidden temptest running through the blood, into the heart, of someone who shouldn't have such. The forbidden thing here, though, is their love so it pretty much symbolizes how their love can destroy the two of them. Unless you're not really religious, then I guess it's just kinda a picture. =p
