This is a flashblack of the time period in the first game. This is Alice in the Rutledge asylum, when she was REALLY mad. The whole first game was played entirely in wonderland. Alice was never self aware the whole game.

When I first saw the asylum area in another walkthrough, I was confused because I thought time had skipped in-game... then the part with the lobotomy flash-back happened...
