Well, Vova's throw was not accurate enough, but why? Probably because he got not accurate enough throw from Olga right before he threw that club. He still could catch it, but couldn't make an accurate throw.

Its hard to say. They both did bad throws from time to time, but that was corrected by other passer. Why Vova did a bad throw - I don't know, maybe because Olga did it first, and Vova couldn't correct it.
