I looked up the identity of the caller on the Silent Hill wikia and it's possible that the caller is just a manifestation of Silent Hill like Maria was a manifestation in Silent Hill 2.

In the crematorium there are the stretchers of corpses with numbers on the sheets, #7 is Stanley who has been dead this whole time. You can sometimes even hear him choking when you go close to his corpse.

So they're the same girl, but different ages with different experiences. And now there's Heather. That's a lot of lives for that poor girl to remember. XD

The whole birthday phone call has to do with the ages of Cheryl, Heather and Alessa.

When the mysterious caller said Happy 31st Birthday that was the ages of Alessa (age 14 when Silent Hill 1 took place) and Heather's (age 17 during SH3) added together.

When he said Happy 24th birthday that was Heather and Cheryl's (age 7 when Silent Hill 1 took place ages added together.
