Just in case you're still wondering, Icran, that death by flies thingy is basically the ultimate punishment for not tidying up after yourself. If you let 15 stinking, fly-infested items pile up, there's a chance that when a sim walks through the breeding ground that they'll form a killer swarm and eat the sim alive, as you may have seen.
