there should be a link that says something like "copyright options" in an email YouTube should've sent to you via email address. if not, go to My Videos, then click the video, click the copyright link, then click dispute. It should come up with a whole page of stuff, when it gives you 3 options about the reason for dispute, click the 2nd option and type in some sort of disclaimer, put your proper name in the signature box, then type out manually the statement of good faith, dont c&p.
