well when u kill someone thats not ur target, u get a pickup penalty. If you pickup the drop, youll get a cant leave penalty where u cant leave for 3 mins. Why they leave the cave is so they can get better defensive gear and more food/pots (super restores and saradomin brews etc)then they return to the drop and hope they can get out

I'm actually not sure myself since I don't pk, but the last time i checked, once you get out of the cave, you have to wait 30 seconds to get back in. This means that:

The time it takes to get from your kill to cave exit+ 30 seconds (possibly more considering what you do in the bank) to get back in the cave + The time it takes to get back to kill, and considering you end up at any random location, it can be very hard. I think items disappear after 2 minutes though, so it's not too bad.
