If you see the special features on the DVD, they show the filming of the amputation scene, which was attended by representatives of Fangoria magazine, a publication that tends to cover horror films, especially their gory special effects. Taken out of context, I believe there was laughter during the filming of the scene.

(i just finished the book, so i have a pretty perfect expl. xD) anywho, due to the drug use, he has trouble getting veins to the skin to puncture them, and keeps goin to the same spot, which turns to a hole in his arm. he keeps using it and doesnt clean it, and it gets infected.
