The way I interpret it is the people who you see, clinging onto their drips and in nothing but a nightie and slippers, looking so ill, their life slowly fading away. But still they're dying for a cigarette - almost literally. It's a very sad thing to see, that they've caused their illness but at the same time they can do nothing to stop it, so it becomes a viscous circle which they cannot amend. That, or the smokers outside can be in good health but almost each one will end up in the hospital.

Is this truly a live performance? The video seems too perfect - multiple cameras, panning, zooming, etc. that isn't common for live tapes. The vocals are amazing btw :)

Hahaha! I think the editor done a good job! I'm not a editor but if you look carefully in the begining he starts with the cameras that are further away and he slowly comes closer to the stage, I think he puts singers voice in value, his powerfull voice. Idk it gives you the feeling that you're there but in a further corner and suddenly the music capture you're attention and makes you come closer, because you enjoy the singers voice and their music. Watch the cameras move with the music tempos.

Well, a hospital takes care of sick people, and smoking can make you sick. So it's pretty sad to stand outside the hospital doors and smoke at the same time, I guess :)
