I understand it deeply as an orthodontist. This is a good video. That is why I alert my patients to go to general dentist every 3 months. Diagnodent pen can't indicate caries decay under dental sealant. Thank you very much.
Youtube is full of KIDS that reply to your video. My advise just let them think and speculate you did one hell of a job with that tooth. Well done! very professional.
I had ten sealants put in today on each tooth today. I had one resealed because one of them was growing worn and bore a brown stain that made me paranoid. But now it's good as new. :D But now I'm scared of them failing to protect my teeth. =/
@sandyski100 I'll answer this question for the last time. if you are a dentist you can see that this person doesn't have a 2nd molar which usually comes in at the age of 12. I suppose it could be a 30 y/o who's 2nd molar was removed long ago. I made the video. I have no reason to make up the persons age. It wouldn't matter if the kid was 6 or 16, there's still decay under the sealant. As for the facial hair, what would you like me to say. What would I gain by making up this person age?
@sandyski100 I treat a lot of kids. it seems as if kids today mature a lot faster than they use to. Next time you're out and about look at some of these kids. I have young teenage patients that have the same shoe size as me and I wear a 12. Also I filmed this under a microscope. You wouldn't believe how many women have facial hair when viewed under a scope. I apologize for my last reply, but I just can't see how I would benefit by making up this patients age.
where's this facial hair you speak of? oh you mean the peach fuzz only around the lower lip? certain ethnic groups are hairier than others. maybe this kid is from the middle east. if he did have facial hair then he would have stubble on his upper lip dumbass.
It's been a while since I posted it this and I was thinking out loud as I typed the post. I removed a good portion of the distal lingual cups and had to undercut the enamel. I know from experience that in the future this will be a area that can fail and if it does I would probably need to place a onlay or a crown. I did not mean that the tooth would need a crown immediately. I apologize for the confusion
You cannot seal over decay. The cavity will continue to grow. This appears what has happen in this case. The tooth must be caries free prior to placing a sealant. Use a diagnodent prior to placing a sealant.
When I place a preventive restoration I use a dental operating microscope, air abrasion(to make sure there is NO DECAY prior to placement, and I use a higher filled material than a traditional sealant material). That's just me. I'm not here to debate about others techniques, this is just the way I do it.
It seems as if there are 2 sides of the fence about sealants. It seems as if you choose not to do them you are considered an outcast. I'm glad to see other practitioners other than my circle of friends who share my thoughts about sealants. Thanks for the comment!
@EdmondSmiles im a dental student so quite interested in methods, if sealants are bad or easy to place wrongly what would you surgest as an altenative? :-)
No need for me to lie. Any dentist could see that this child does not have the 2nd molar or 12 year molar. That would get the tooth that you get around the age of 12. The tooth I was working on was #3 or 6year molar. Anyone can see that there is no tooth distal to the tooth I was working on. Distal would be behind the tooth. As far as the facial hair, kids mature faster now. This was done under a scope so every detail can be seen. It would be nice to ask before posting negative comments.
So true Dr. I am a hygienist of 11 yrs married to a dentist of 17 yrs (with facial hair) who has NEVER placed a sealant. Despite almost getting kicked out of Loyola and not to mention loss of financial gain. Principles stand true!!

Why does the info state the tooth will need a crown? Was the restoration much larger than it appeared?

I agree 100%. But I'm certain using a diagnodent is not common practice.

Sealants placed under chewing pressure are forced deep inside pits and fissures
of upper and lower teeth at once and should last longer.
However it is best to have a dental check up every year to make sure your tooth care is adequate.
