The chords are D,G 6/9 ,A sus4 and Bm . Not sure if it's correct but it seems pretty close . PS ( G6/9 = bar 3rd fret , middle finger 3rd string 4th fret, ring finger 4th string 5th fret and the A sus4 chord is a D chord without the middle finger ).
