I can't wait for this game, it looks so freaking awesome!! and Naruto Ultimate Ninja 3 wont be out in the US for a long time, the second one doesn't even come out for a few more months. and uh, no, the game wont change if you mod your PS2 :P it only allows you to play it. You can usually find translations and guides online though. my friend is importing and he's giving me a copy! check out the magic swap disc if you don't want to mod your PS2.
