After some time in the lungs, it spreads to extrapulmonary tissues with a predilection for the brain causing meningoencephalitis . If untreated, cryptococcal meningoencephalitis is 100% fatal, and even when treated with the most effective antifungal drugs, itcan be fatal if the host does not have adequate T-cell function
C. neoformans var. neoformans causes most cryptococcal infections in humans and is found worldwide; its main habitats are debris around pigeon roosts and soil contaminated with decaying pigeon or chicken droppings (1,3). It is generally accepted that the organism enters the host by the respiratory route in the form of a dehydrated haploid yeast or as basidiospores.
Cryptococcus neoformans is an encapsulated fungal organism that can cause disease in both immunocompetent and immunocompromised, hosts . Most susceptible to infection are patients with T-cell deficiencies .
For mild oral thrush, the usual treatment that is tried first is miconazole mouth gel for seven days. Sometimes a two-week course is needed. Nystatin drops/suspension are another option if miconazole gel cannot be used (for example, if you are known to be allergic to it). Amphotericin lozengess are anotheyr option.
Oesophageal candidiasis causes difficulty in swallowing and pain while swallowing. Treatment is with oral luconazole , ketoconazole, or itraconazole.
Pneumocystis pneumonia in those with AIDS usually develops slowly over days to weeks or even months, and is less severe than when it occurs in other forms of immunosuppression. People with pneumocystis pneumonia who do not have AIDS usually get sick faster and are more acutely ill.
Symptoms include:
Cough -- often mild and dry
Fever
Rapid breathing
Shortness of breath -- especially with activity (exertion)
Good coverage but speaking too fast somewhat kills the mood. It's better to have a constant structure for covering each illness like say cause, symptoms and signs , investigations/diagnostic criteria and management
