"When you live in a world where some people are dying from too much food while others are dying from not enough, maybe having your love life being manipulated by an audience isn't the most ludicrous thing going on"

Well, we're watching these snippets of his life in 7 minute weekly increments. I don't think they have that much time to delve into the psychological ramifications of reality-altering and still be entertaining.

So, yes, it requires a bit of suspension of disbelief. The very idea that there are cameras everywhere in the city set up by some unseen manipulator allowing us to spy on the protagonist lends itself to the fact that this isn't grounded in reality.
