The episode of The Nightmare Room called Scareful What You Wish For- Shia Lebeuf puts away his toys but his childhood doll Buddy follows him, causing havoc. Near the end of the ep, Shia rips off Buddy (the doll)'s head but Buddy (unseen by the audience) somehow puts it back on. Thus there are shots with Cole/Dylan with a proper neck and then with prosthetics to appear as though his head has been hastily reattached.
