That is a very good question. Hotmail are a right pain and do not support IMAP, only POP3. If you only want to use POP then go to settings, mail, add account, other then enter your details such as email address etc.
If you want to use IMAP then you have to use something to do the IMAP translation for you. To do that look into something like izymail (search on Google). It costs money but only about £12 a year.

That is then linked to you iPhone and that is how you are billed for apps. You then get a receipt sent to your email (usually the next day) with all your purchases made the previous day and are billed automatically.
