@IBealo The bang is a certain decibel level, we can stand so much, the human auditory threshold for pain is about 120-140 decibel's, the bang in the grenade well exceeds that. The pressure of the sound wave throws the fluid that maintains balance in the ear off and most people just drop on the spot. In a fight a person can do the same by cupping the hands and smacking the persons ears hard as possible. Not only will it likely break their eardrums, they'll drop like a rock.
@IBealo Because it's only used against (or supposed to be used against) extremely dangerous people. the risk if knocking a few hostages off balance to save their lives seems worth it. Discomfort is over-ridden by mortality in most cases. Plus, saves them having dead perps, cuffing them when they're on the ground trying to figure out if they need to hold onto the carpet to keep from flying away is easy.
@IBealo Because dealing with some tinnitus, dizziness and spots in your vision for a week or so is generally preferable to having your viscera sprayed all over the stairwell by a well placed shotgun blast. It's meant to cause intense, overpowering discomfort...they have to deal with the bad guys somehow, so their options are either go in guns blazing and sort the bodies out later, or try to disable everyone in a non-lethal manner and then deal with the criminal and living victims.
