In the before side pic, he has tight underwear pushing the loose skin creating that flap you all want us to be amazed is not there in the 'after' shot. However in the after shot he has no underwear to push the skin, and the pants are covering the extra loose skin at the front.

In the front shots you show his before/after shot. The after shot CLEARLY has his pants pulling up over his loose skin.

In the before shot of his side view, you cannot see the man's armpit. In the after shot you can - thus it's safe to assume his arms are horizontally stretched in-front of him for, lending to a saggier look.

In the after shot you can see his arm-pit, therefore arms are vertically stretched in the air, hence pulling the skin upwards, appearing tighter.
