The title isn't just a sexual reference. It's ambiguous. The song is mostly about death. The dirty hole is actually the earth. Think of a the image of a grave; a dirty hole. "How many dead men, God? ...How many men have died in your dirty hole?" The song starts off by saying "I saw the gravestones. I saw 9 year old boys." He starts of contemplating the cruelty of death and the anger at a creator that would allow such atrocities.

Calm down, dude. I said it's ambiguous. Meaning it has two meanings. It can be taken two different ways. Besides, the line "my life flashes before my eyes..." can be directly related to death. What do people often say happens right before death? That's right. Your life flashes before your eyes. And asking "how can the earth be a killing hole?" is probably one of the stupidest questions I've ever heard.

I first heard this track around ten years ago, and liked it straight away. But I have always wiondered what it is about...is the 'hole' he sings about an allegory for death? A reference to the grave, or just a reference to mortality in general? Perhaps something entirely different?...And who is he addressing--God? The universe? A person?
