It's impossible to warp underwater because in wolf form, you can't dive. And you can't warp while swimming either.

This happened to me once, when I drowned in the fishing pond. The difference though was that I wasn't given a chance to retry (I can't hear Audio in safe mode, are you still playing in the GameOver screen?) so I couldn't save or anything
