Hopefully this will get rid of all speculation. I know the bridge operator and his family personally. He was sick and on multiple medications which severely altered his state of alertness. He was called in to work that day on forced overtime (work or you're fired). Some responsability does fall on the operator however a very large portion falls on the Seaway due to making him work in unfit conditions... The labor code now protects employees from unsafe work

Everyone survived. They had plenty of time to get out of harms way. The accident was due to the person who operates the bridge falling asleep. The boat could not slow down due to it's massive weight.
