I think so too about the name but for some reason they were uncomfortable with the word "Death" so they used Slade.
It makes me wonder why worry about that if later on a godlike demon turns Earth into Hell. You'd think that would upset them lol

This is totally cool but I'm gonna answer some fo those questions.
1) THere is a mualed face behind the mask. Slade aka Wade Wilson's sife sho thim in the eye.
2) His became evil when men kidnapped his son(who you know as Jericho) and slit his throat making him mute.
