The character's full name (as given at any point on screen) is Doctor Leonard H. McCoy. "Bones" is a nickname he was always called by Kirk. In the series there's evidence to support it is derived from "sawbones", which is a slang term for a surgeon. In the movie, as you saw, his nickname came about in a slightly different manner (not to say the original derivation is not still applicable).

He was in the very first TNG episode, the 2-hr pilot, "Encounter At Farpoint". He arrived at Farpoint via the USS Hood and came aboard the Enterprise for a tour of the new ship. We never saw the tour, but we did see Data escorting him to the shuttlebay, so he could take a shuttle back to the Hood, which is where the closing scene of this video is from.

I assume you mean from the same scene? It would have been Kirk. McCoy had just finished a physical examination on him when he said that line.

AGREED.
The one thing that Karl Urban doesn't have is those gorgeous blue eyes. McCoy was the most beautiful person on the Enterprise.
