I found to get rid of the last 5% (on ply-wood) is extremely difficult. So a buddy of mine told me to buy a real cheap 4.5" angle grinder and get a bunch of sanding pads (coarser the better), and sand away. It worked great now you will have a nice level surface for you new flooring.
tore up 2 3ft by 4ft sections of tile in my gfs house, first section took over 4 hours using a heavy duty weapon i made with the cutting edge of a bulldozer blade with a big pin welded to it to try and beat under it with a sledge hammer.....the 2nd section i rented a (boschhammer) like this guy, another common brand is (hilti) they use SDS bits, they are heavy, but considering what you are peeling up with them, your much better using these things....the second section took less than 5 mins ;)
