Oh man! USE FRESH NEEDLES! You can see how much force he's using trying to get the needle through the skin, that means the needle tip is totally blunt and barbed. If you have to use any IV, IM or SC drugs, use a clean, fresh needle every time, and wipe the injection site with an alcohol swab beforehand.
