Having kept them for years I can guarantee you they don't recognise your face or voice.They can appear to look you in the eye but really their just responding to movement such as a blink as it resembles the movement of an insect and their always on the look out for prey.To give you an idea I saw one eating an ant once while other ants swarmed over it.It kept eating even though the ants were killing it.Not bright.I'd say they don't even know their sitting on a person and see us as mere objects.

The reason she was getting weaker is because she didn't have the right set up or food (judging by your description). Mantises need lots of foliage and sticks to climb on. House flies would not be big enough prey for her, large crickets would have been much better. Also, too much handling would stress her out.
I noticed she was an adult and had a few bits of damage.They only live a few months once full grown so she may just be reaching the natural end of her life.They go sluggish and then drop off their perch.
These are such great pets, if you handle them regulary for about 2-3 weeks, they will be fine with you. Mine has never bit me with the 7 months i've had her :)

I used to have an Orchid Mantis. Those things are absolutely beautiful. They vary of color but mostly white and look like a part of a flower.
