I managed to make a SCARE lead for my plus 3a a few years ago by using the pin outs in the back of the manual.
The reason being the poor sound output the plus 3 had from the factory :(
Anyway I noticed you're using the sound output socket of your speccy in the scart lead. You don't need to do this, the sound should be fine out of the din monitor plug.
I hooked my +2b up to my new 32" LCD TV and it looks shite. Looked amazinglt crisp and sharp on my old 28" philips CRT. Now u can see vertical columns in the background and the characters are all blocky and rough. Any ideas mate?
Probably your TV uses the settings for the normal TV receiving, and since TV channels have a different resolution as a computer from 1983, you might search in the menu for an option like "Factory Reset" or "Realign" or "Auto Align".

Hi,the yellow connector is RCA plug connected to TV out to give better ground. The sound come from monitor connector.
