The music is from a common licensed album called "ghost". It was available for free download from the nin homepage for a while. You could always get it from pirate bay i guess since it is common licensed.

Well, I dont know what to answer on that one. You can se pretty clearly that the inspiration and the reference photo for the head profile comes from the video. But it's not really directly taken from the video. Looked at a lot of different photo's. To say "this is taken right of the bat from this video" is a bit unfair and inaccurate.
