This part is only tricky if you play it on Hard, or Very Hard, because he switches direction mid-swing. So, even if you're out of the knife's trajectory you can still get sliced. Otherwise you shouldn't have a problem.

I've been trying to put alllll the pieces together from all the Silent Hill games. Got info that The Butcher from Silent Hill: Origins is the 'first stage' of pyramid-head. Therefore this means that Pyramid-head = Travis (the protagonist of SH:O) because of the bad ending and what he did...
