I would say he used masking of course first to keep the rest of the hand from moving....then simply used the move tool.

did you do ALL this in zbrush without modeling anything in maya or 3ds max before hand?

as i can remember on the video that you show the printed soldiers, they had a octogon shape at the bottom, but in this video it has a more round shape. Did you change the base before sending it to the printer, or?
