It was meant as an easter egg as a way to add the chief to the game so it didn't have anything to do with the story line and in the scene from Reach you mention you can see that frigate half in the background during the glassing so it can't be the one from Halo 3. I also don't think it's Reach because Cortana wouldn't have said she didn't know where they were then.

in the last few seconds of the video you can see the surface of that planet. After much planet comparing, I believe that the planet is Reach after the Covenant attack.
