Bionic Commando was one game that truly freaked me out when I was younger. You have to fight a resurrected Hitler, which is scary enough, but when you defeat him his head blows up with gore and brains everywhere. Nice list. I knew about some of these like the Joe & Mac one but didn't know about some of the others. I always thought the bug monster in the poison forest in Crystalis was sick, but he pales in comparison to some of these.

A Nightmare on Elm Street had some pretty grotesque bosses. Some of the most f'd up characters were in those Japanese Qix-like porn games like Miss World '96 where a picture of a hot girl would turn into a Hellraiser-like demonic creature.

When watching this for the first time, I paused it early on an thought about some games and the 3rd level of Shinobi 3 was one of them. You blew my mind with that one. As a kid when I played the game at night, I used to turn it off when I got to the 3rd level. Now, I have no problem with it but back then, it took me some effort to play through it. Once I got past 3-1, it wasn't a problem with 3-2 & 3-3. The boss isn't the hardest but he's the creepiest in the game I think.

Although I've never played it, I've found that the stage 1 boss for Contra: Shattered Soldier is mildly disturbing. It's basically the first boss from Contra III: The Alien Wars, but much more grotesque (especially when it turns its back to you).

I ended up playing most of those games, and I must say, great picks. There was one boss that I think was missing though: The Nightmare boss from Metroid Fusion. I'm sure anyone who's played it could agree with me on this one. The game wasn't overall scary, but that one boss was just... freakishly awesome. The face starts melting off as you fight it and it slowly turns to a green slime, which you then have to destroy.

Athough he isn't gory, or disgusting, Giygas from Earthbound is pretty damn creepy.... the only music that plays sounds like echos and breathing, the room giygas waits in resembles intestines, with a large eye at it's centre, which changes to look like you main character(ness)'s face. It's particularly scary since the rest of earthbound is a lighthearted, cheery, funny game with great music, and charming graphics.

Also, I think giygas's 'face' looks a bit like splatterhouse's final boss.
