It seems so incomplete, a lot of questions unanswered and a lot of potential story elements undeveloped. I think the reason he died was, When he got the Moonshadow (Sword) he was scarred in his soul, with his nightmares (Or Visions) Getting worse every passing day, hurting his soul more and more, until it just collaspes and he dies. Of course from the preview of chapter 3 we can see he may be coming back.
