In addition to a myriad of other professionals on board (in ANY professional film or theater endeavor), the director provides the necessary information about era details (such as clothing, language, geography and of course hair style). Holmes had a personality that could find anything in a mess as long as it was "his" mess if that makes sense. I wouldn't necessarily call him a 'slob' though.

And he was addicted to cocaine, a seven-percent solution as he states in [I think, don't quote me] either a Scandal in Bohemia or the Sign of the Four. Either way he breaks it by the end of the entire series. He also did morphine on occasion but that was only mentioned in passing.
