Definitely comes from a nihilistic and realists point of view. It's basically saying there is no truth. Everything is all interpretation, and that the only truth is, is that everything he mentions just IS, it exists, nothing more. Everything is labeled as something by us, but what we label it, call it, or what we say whatever "it" is, is just an interpretation/opinion/assumpt­ion. And the whole song he's pointing out how everything changes, which is meant as evidence to prove his ending notion.

What is changing is the persons life throughout the song and it deteriorates into a meaningless and nothingness existence.
