I've never read the actual cause of Billy's death, but I'd read that he had been having serious health problems since he suffered a ruptured ulcer in 2005 and almost died then. He needed stomach surgery, was in a coma, had pneumonia, and recovery was slow. Maybe it relates to that.
The voice on the original recording was George Cummings, who shared a songwriting credit with Shel Silverstein on this one (you can also hear George sing the second verse of "The Cover of the Rolling Stone"). Billy Francis took over the lead vocal in live performance on this song after George left the band.
