For me it depends on the relationship - when a straight guy says to a straight girl (or vice versa), 'let's have sex', there's usually no doubt that they're talking about penetration, but in a lesbian relationship (in my opinion) the word 'sex' would refer to any kind of sex - fingering, oral sex, maybe even penetration using sex toys. I can't speak for gay men, but I would think that in that kind of relationship 'sex' is usually understood as anal sex, though like I said I can't really know.
