Simply, it means you failed a flight -- the things you were supposed to know/be able to accomplish, you didn't/couldn't. Hook enough, and you're flying with the CO for an elimination check. Hook that ride, and you're out.

Ok so what does it mean to "hook"? I am assuming it has something to do with flying with the flight commander which is bad news.
