Dr. Destiny (called Dee Dee by many fans) trapped the league in their worst nightmares but Martian Manhunter was too strong and Batman just was.. he's the goddamn batman who as he says "my minds not a nice place to be Johhny." I think he left him with acute chiroptophobia.
Superman's worst fear is that his powers destroy those around him. But it's also been that he'll fail to save the day and everyone will die - except him. It doesn't help that in multiple universes he is the sole survivor of some Doomsday (pun!) event or another.
