I do believe it was 1976's "The Hand of Fear" (I got the clip off another website, and I don't have the DVD yet to confirm this)

Yes, it was "Hand of Fear" which was Sarah Jane Smith (Elisabeth Sladen's) last appearance as a regular travelling companion with the Doctor. The next episode is just the Doctor - only one the whole first run of the series with no companion, and then Leela starts her trip.
