@goldensilverstar He planted an idea in her head that made her believe that the reality was actually a dream and that the dream was the reality. He and Mal went to sleep the same way that every else has been doing it in the movie (the monitor to put them to sleep) and they grew old together in the dream...during that time in the dream he put that idea in her mind so when they finally woke up in reality....she believed the reality to be the dream. Hope that explained it.

@goldensilverstar In the dream world, in order to return to reality you must die. So she kills herself believing that she will return to reality when actually she was already in reality...leading her to actually die.

@lambertlover978 i think it was more toward Mal believing that her Limbo was real. After they spent all that time in Limbo, and thrown back to the past, that's when Mal started questioning her reality. so thats when Cobb went back into her limbo's safe, and planted the idea that Mal's "reality" wasn't real that her Limbo wasn't real. In his eyes, he was trying to save her.

@Karnezar No! Mal really was a damaged. Her past had so much pain that really cob was her only way away from it. she lost her reality, chossing to forget. They themselves were discovering how far you could go, so he had no time realize all aspects of inception. That he had implanted a negative percevied idea. unlike they did to fisher which actually made him a better man. but made mal more damaged than she started, sad! :(

@minxinglee See, she's gone mad, but it was because he drove her to it. First time through the film i felt like Mal was the villain. Upon subsequent viewings, knowing what know, i actually find her incredibly sympathetic. She was lost, he made a decision to act, and they both suffered the consequences.

@TheSandyNub 3d makes more money, unfortunately. More expansive tickets and more people go to the cinema because they can't recreate 3d at home (properly, at the moment). That's how shitty movies like clash of the titans are able to make a shitload of money.

If they were in a dream for 50 years, why did they grow old ? Also, they could have created anything in their dream, the most wonderful place....and they decided to create a sterile, cold, repetitive world of anonymous concrete skyscrapers ? Now is that the stuff "dreams are made of" ? I don't know.

@hudson0815 why would they grow old if in their minds they didnt know about growing old...only reason Saito is old in his limbo is because he entered it with the mind state that he was old and dead. this was just their reality where they could create anything, time being non-existent to them

There's a very short scene showing them as an old couple. For some reason, Cobbs hands looked young though. I guess it was a mistake, since his hair was white...

@DeLaSoleil No, no, no. She was so happy down there in their dream world that she planted the idea in her own mind that it was real. And he knew that it wasn't and he wanted to get back to their real life in the real world. So when he planted the idea that "this world isn't real," he hoped it would just make her realize that the world they were in at that moment wasn't real and he had no idea it would stay with her when they returned to the actual real world and cause her to seek out a way home.

The beautiful thing about this scene is when you stop and realize that mal is just in his dream, and that he is not revealing the truth to her, so much as he is confronting the truth himself, head on... Ideas are powerful, and the most powerful ones are ones we keep telling ourselves, in Cobb's case he probably told himself "I can't believe she's dead" and "I caused her to die" "If only I hadn't planted that Idea, then we could have been together forever" causing him to imagine her in his dreams
