What I get from it is it's a gov't institution for torture (possibly secret) and it shows how this man who tortures people daily goes through the same routine as most office workers without a second thought of how strange it is and for him torturing becomes as dull as writing reports as a desk all day but he does it, like office workers do, because he needs to money to get by everyday

I liked his earlier bleak gritty-city stuff (being from the country) but Hospital Music is his defining album/CD. He grew up and became old and fucked up just like the rest of us. He sings for all of us now. But I'll have to check this old one out.

I don't think the joke itself is worth delving into. I don't think it has any relevance to the video other than the fact that people have an uncanny ability to separate themselves from what they do for a living, and what they would wish upon the people they care about. It's just thrown in there to illustrate the separation. If the joke made any sense at all, then you'd be thinking about the joke, and not the point of the song.
