And before anyone says anything, allow me to explain myself. In the manga, Shion had this same exact hallucination of Mion crawling into her bed. It was... really creepy. Besides, Keiichi didn't know that it was Shion, so the hallucination would be Mion, as that was what he believed.

So after Ooishi gave the news that he found Mion's body at the bottom of the well and left, he had a hallucination of Mion's possessed dead body coming into his hospital room and killing him. He dies of a heart attack because of it.
